Treat an answer as input, not as a final instruction

When a client answers a questionnaire, decide whether it is a fact to store, a preference to explore, or a decision that needs approval. A response hidden in a form can become stale or be interpreted differently by different people. A decision record makes the chosen option, owner and impact visible.

This is the follow-on to a staged questionnaire system: ask at the right time, then move material answers into the record that drives work.

Use one decision record

FieldExample
QuestionWhere will the ceremony move if rain is forecast?
Optionscovered terrace; ballroom; proceed outdoors
Ownerplanner prepares options; couple approves
Decision datefictional: 15 August 2027
Chosen option and reasoncovered terrace; preserves access route
Affected workseating, signage, musician arrival, guest message
Confirmationvenue confirms setup in writing
Statusproposed / approved / communicated / complete

The record should not pretend that every choice is permanent. If a material condition changes, update the decision and say what changed rather than silently overwriting the earlier rationale.

Worked example

In a fictional design questionnaire, a couple selects “minimal floral colour.” The planner records that as a preference, not a florist instruction. After the venue visit, the planner presents two arrangements that fit the light and budget. The couple approves one. The decision record captures the arrangement, the budget effect and the installation point; the vendor brief and timeline then receive the current instruction.

Keep approval separate from a message thread

Messages are useful for conversation. They are a weak place to find the final answer weeks later. Send a concise decision summary after the discussion, name the approval route, and update the source-of-truth record only once that route is complete.

Cakd Business and Agency include questionnaires and decisions. The article’s classification and follow-through are a studio workflow, not an automatic conversion of answers into tasks. Pair it with a client communication charter so clients know where an approval belongs.
